Weekly Wellness Checks

This is a weekly service with a goal to keep your loved ones out of the hospital long-term. I will keep an eye on your family member(s) or loved ones and update you weekly as needed. As a member, you will receive the following services:

• 1 hour weekly in home visit/wellness check (90 minutes for 2 people)
• B/P, Temperature, Heart Rate, Oxygen level
• Heart, lung, circulation, mentation assessments
• Check the home environment for safety
• Review any medical reports, upcoming visits and organize them
• Medication education, pour medications and make a schedule if needed, and review
• review any medical tests
• Education on health issues
• Schedule and transport to appointments if needed
• Communicate with medical team if needed
• Weekly updates to family as needed
• Companionship

Pre-op/ Post-op package 

Package consists of a maximum 4 hours. Time starts at pickup from patients home.

Cancellation Policy

To receive a full refund you must cancel at least 48 hours before your scheduled appointment. Cancellations less than 48 hours before a scheduled appointment will result in a $150 late cancellation fee. Note that the cancellation policy does not apply to canceled or rescheduled procedures.
